Piercing Minors

Paperwork

For any procedure involving a minor, Michigan State Law requires that the parent/guardian be present to sign the paperwork, remain present during the procedure, and provide to the studio their unexpired photo id, and the birth certificate of the minor

  • If minor has a form of personal photo ID (school ID or drivers license), please bring that as well!

Forms of id

For the parent/guardian, one of the following:

  • Driver’s License, State ID, Military ID, US Passport / Passport Card/ Guardianship Paperwork

  • From the minor:

    • Birth Certificate (We can accept an unaltered photocopy)

    • Bonus Points – Student ID, State ID, College ID, US Passport, Passport Card
